If a student diver progresses from one course to the next within a relatively short period, must he fill out another medical form?
Yes. Have student divers complete a PADI Medical Statement
(RSTC Medical Form) prior to every course (except Emergency
First Response and non diving specialties).
If the student diver in Question 2 answers “yes” to any medical questions, does he need to obtain medical clearance from a physician?
Only if his medical condition has changed since he completed
the prior medical form. If the student obtained medical clearance
from a physician within the past year for the same “yes”
answers, he does not need to obtain new clearance. However, if
he answered “yes” to any additional questions, he must receive
medical clearance from a physician prior to any in-water activities
for the new course.
If a student diver completes one course and immediately enrolls in another course, must he complete a new liability release form?
Yes. Because each course may expose the student diver to different potential risks, the student needs to understand those risks,assume responsibility and release the instructor from liability by completing a new form.
Exceptions include newly certified Open Water Divers who participate in an Adventure Dive on the same day as Open Water Diver Course Training Dives 3 and 4. Also, when a student enrollsin a PADI Advanced Open Water program and completes the PADI Liability Release and Assumption of Risk Agreement for Advanced Open Water program and Correlating Specialty Diver programs. This release covers the Advanced Open Water dives and extends to subsequent correlating specialty diver course
dives.
